• Assess the needs of the elder
  (The assessment will be done by a registered nurse with a master's degree in advanced medical-surgical nursing)
• Develop a plan of care with the elder and family participating as partners in the decision making
• Provide information/interventions and options
• Be an educational resource
• The professional geriatric care manager will act as an advocate for the elder seeking their highest good and maximum functional potential.
